Internship programs are an effective way for HT students and professionals to gain "field experience" and learn more about the theory and application of horticultural therapy as a treatment for people with disabilities. Interns typically work as a functioning member of a department or facility, under the supervision of a registered horticultural therapist at the HTR or HTM level. Periodic evaluations monitor the intern's progress toward specific goals, such as developing effective communication skills, ability to organize programs, and developing an understanding of the psychology of disability. Interns may be eligible for .25 points toward professional registration upon completion of an internship; to earn points, the internship must be supervised by a professional at the HTR or HTM level.

AHTA provides detailed guidelines on goals, policies, and procedures that may be followed to provide an effective internship. These guidelines are suggestions only – AHTA does not accredit internships and institutions are under no obligation to follow these guidelines.
